Community Health Assessment and Improvement Plan
The Lexington-Fayette County Health Department, in partnership with the Lexington Community Health Improvement Partnership (LEX-CHIP), is proud to share the 2023 Community Health Assessment and the 2025-2030 Community Health Improvement Plan. Theses documents take a look at current health trends through multiple sets of lenses. They use the information to inform decision-making for community engagement and partnerships to improve the overall health of the community.
