PATH Mini Grant

LFCHD is excited to launch a new Community Health Improvement Plan (CHIP) mini-grant initiative: PATH (Partnerships Advancing Transformation and Health).

PATH is designed to invest in community partnerships and locally driven solutions that address priority health needs identified through the CHIP. This mini-grant opportunity will support short-term, community-led projects aligned with the following CHIP priority areas:

  • Access to Care
  • Financial Stability
  • Knowledge Empowerment
  • Mental Well-Being

Priority consideration will be given to projects serving residents in ZIP codes 40504, 40508, 40511, and 40517. PATH applications will be open throughout the month of January, and eligible organizations may apply for up to $5,000 to support a community-led health initiative.

Applications and supporting materials will also be shared through LFCHD social media channels and the LFCHD website.
