Download the Wellness Passport below and complete ten (10) wellness activities from the Wellness Passport and document on the online form (link below) to be entered into a prize drawing!

To be eligible to win prizes, participants MUST take a photo during each activity for proof of completion.  Note that you will only need to provide photo documentation if your name is drawn for a prize.  Be sure to take pictures of your activities and save them for proof if you are drawn for a prize!

Participation in Wellness Passport activities are free of charge, open to the public, and are the choice of the participant.  Lexington-Fayette County Health Department shall not be responsible for any injuries or losses sustained from participation in Wellness Passport activities
